The public funeral will take place on Nov. 21, 2015, at Shepherd Church in Porter Ranch, Calif. Following the funeral service, people will gather at Burial Forest Lawn Hollywood Hills. In lieu of flowers, the family is requesting donations that will help and hopes to meet those who have contacted his family to hear their “ideas to keep Sam’s memory alive.”
